Defect: On certain trailers the welds are breaking allowing the unit to move resulting in the inserts being pulled freeing the unit from its mountings. Or the inserts are pulling out loosening the remaining fasteners and freeing the unit from its mountings.
Remedy: Inspect and if necessary modify the trailer's fittings and fixtures.
